How to file a claim for

This is applicable to PREMIER THE ONEcierge One Team Health Management network doctor

We want to help make the claims process as quick and easy as possible. So please read and carefully note the following important information:

  1. Please submit an application form at least four days prior to when you want to be treated
  2. Please check your policy to ensure your illness / disability is covered
  3. Please check that your policy covers the treatment fees requested
  4. Before we organise any cashless medical treatment, we’ll need to check your account to ensure there are no outstanding monies owed to Bolttech so we advise that you settle any outstanding bill(s) in advance

Remarks: Complete part 1 to find an appointed specialist. Don’t worry about part 2, your appointed specialist will complete this.

We will get back to you within one working day to advise if your application has been successful. Your appointed specialist will then explain the medical costs and exactly how the service will work. They will also arrange your hospital appointment, at a time to suit you, at the hospital you’ve chosen. Note that you’ll need to pay any outstanding deductible costs at the hospital.

If we need additional information from you, we will let you know – either by post or via your agent if applicable.
