If the insured is a Hong Kong permanent resident and the loss happened in Hong Kong:	If the insured is a Hong Kong permanent resident and the loss happened overseas, besides #1-6, we also need:	If the insured is not a Hong Kong permanent resident (i.e. China resident), besides #1-6 and #7-8 (if applicable), we also need:
Copy of insured and beneficiary’s ID Original certificate of death Beneficiaries Designation Form (If there are no beneficiaries, please provide letters of administration) Copy of employment letter of the deceased Copy of the last three months' salary of the deceased. For example, pay slips from the employer Guardianship paper (only if the beneficiary is under 18)	7. HKID cancellation proof 8. Notarial certificate issued by the local Notary Public Office	9. Original documents of ‘Cancellation of household booklet’ or 'Cancellation of legal identity/citizenship'
