To be completed by the insured person:

  1. Crisis illness claim form
  2. Declaration and Authorisation for Living Claims

To be completed by the attending doctor:

3. The doctor claim form may vary depending on your diagnosis. Please reach out to your HR department to obtain the necessary attending doctor statement form.

Supporting documents (Copies)
  • Your Identity card
  • Proof of last three month’s salary
  • Continuous sick leave certificates (in other words, the certificate should indicate one unbroken length of time that you’ve been unable to work due to sickness)
  • Employment letter
All medical reports copies
  • Laboratory reports
  • All medical reports (e.g. Surgery report, pathological report, etc.)
  • Diagnostic imaging (MRI, CT scan, PET, etc.)
