The provision of such service and your acceptance of the same shall constitute a separate contract between you and the third party service provider and does not form part of the Terms and Benefits of vPrime Medical Plan. The promotion period is valid from 1 October 2025 to 29 December 2025 (both dates inclusive)(“Promotion Period”).
Designated insurance products include the basic plans of the following individual insurance products that are underwritten by FWD Life Insurance (Bermuda) Company Limited (incorporated in Bermuda with limited liability) (“FWD Life”). The respective Designated Insurance Products are subject to change as determined by FWD Life from time to time without further notice. The policies of Designated Insurance Plans are subject to the respective policies’ terms and conditions. Please refer to the product brochures and policy provision for details.

The Offer is only applicable if the following requirements are satisfied: (“Eligible Customer(s)”)
a. Applicant(s) must submit application of Designated Insurance Product(s) underwritten by FWD Life through FWD online platform with designated promo code during Promotion Period; and
b. The application of such Designated Insurance Product(s) must be accepted by FWD Life and issued on or before 31 January 2026
Eligible Customer(s) must be the Policyholder(s) of the Designated Insurance Product(s) and maintain the policy(ies) of such Designated Insurance Product(s) in force during the usage period of the Offer. Otherwise, FWD Life and/ or FWD Financial Limited (FWD Financial) reserves the rights to cancel his/her eligibility of the Offer without notice.
Each Eligible Customer(s) is entitled to the Offer once only during the Promotion Period.
Unless specified, the Offer cannot be used with other promotions and/or offers.
The application submission date and the issuance date of policy of the Designated Insurance Products are based on the records of FWD Life. FWD Life and/or FWD Financial will not be responsible for any delay, loss, error or unrecognized situation due to computer and/or Internet’s connection, technical problem, malfunction or accident.
The Offer cannot be refunded, changed or redeemed for cash, exchanged for other gifts or services.
If the policyholder cancels or surrenders the policy of the Designated Insurance Product(s) before the end of the first policy year, FWD Life and/or FWD Financial shall have the right to recover the fee of the Offer.
FWD Life and/or FWD Financial reserves its absolution discretion to vary this Offer without further notice.
In case of any dispute arise in the contents of this leaflet or this Offer, FWD Life and/or FWD Financial has final interpretation right on the contents of this leaflet. For any dispute, the decision of FWD Life and/or FWD Financial shall be final.
This leaflet is issued by FWD Life and/or FWD Financial and is intended to be distributed in Hong Kong only and shall not be construed as an offer to sell, a solicitation to buy or the provision of any insurance products or services of FWD Life and/or FWD Financial outside Hong Kong. All selling and application procedures of the insurance plans and services in promotion must be conduct and completed online via FWD Platform in Hong Kong.

The Influenza Vaccine and Vaccination Service under this Offer are provided by Trinity Medical Centre (“Trinity”). FWD Life and FWD Financial are not the service provider of the Influenza Vaccineand and will not accept any liability in relation to the Vaccination Service. Any disputes that may arise from the Influenza Vaccine and Vaccination Service (including but not limited to appointment arrangement, quality/ applicability or supply/ service) shall be resolved between the person who redeems this Offer (“User") and Trinity directly.
Upon satisfying clause 3 of the Terms and Conditions, FWD Financial will issue a redemption letter within 3 months after the issuance of the Designated Insurance Product policy (“Redemption Letter”) to Eligible Customers’ email address registered at application.
The person who redeems this Offer (“User”) is required to make reservation with Trinity and present the Redemption Letter when using this Offer at Trinity. Trinity will verify the entitlement of this Offer by checking the redemption code shown on the Redemption Letter. Eligible Customers should maintain a proper custody of the Redemption Letter. FWD Life and FWD Financial will not re-send or re-issue the Redemption Letter if the redemption code has been used or Eligible Customers fail to use and/or redeem the Offer within the specified time period as stated in the Redemption Letter. Redemption Letter will not be re-issued in any circumstances and the Eligible Customers bear the sole responsibility to maintain a proper custody of the redemption letter
The suitability of the vaccine is subject to the final decision determined by medical professionals in Trinity. If the medical professionals in Trinity confirms that the User is not suitable for the Vaccination, the Offer will no longer be applicable to such User and the Eligible Customer. No compensation in any form will be made to the User and the Eligible Customer.
The medical professionals in Trinity are not FWD Life and FWD Financials’ employees or representatives. FWD Life and FWD Financial shall not be responsible for or liable to their medical opinions, actions, negligence or omission of medical service or treatment.
After enjoying the Offer, if the User chooses to receive extra or other services (if any) at his/her own choice outside the scope of the Offer from Trinity, he/she is required to pay an extra amount to Trinity. The information, price, procedure and arrangements of the Offer on this leaflet are provided by Trinity and for reference only.
For any products and/or services given under the Offer and the use of the products and/or services will be subject to the terms and conditions imposed by the supplier. FWD Financial is not the supplier of products and/or services.
