  • Policyholder's Identity Card.

  • Sick Leave Certificates

  • Physiotherapy / Rehabilitation Progress Report (if available)

  • Termination Letter / Employment Letter by Employer of the Insured (applicable to Waiver of Premium Plus only)

  • Certificate of Assessment (Form 7) or Certificate of Compensation Assessment (Form 5) issued by the Labour Department (if available)

Submission Timeline

  1. Please submit the completed claim form with required supporting documents to us within 90 days after the time the proof is required. We shall provide the necessary forms for filing proof of loss upon receipt of the notice of claim
