Collect supporting documents

  • Medical receipt with patient’s full name, nature of sickness/diagnosis, date of consultation, amount billed for each medical service and the attending doctor’s signature and chop

  • Referral letter from the attending doctor for X-ray and laboratory tests, prescribed medicine and treatment by specialist, physiotherapist or chiropractor*

  • Receipts and Prescription for Chinese Medicine Practitioner’s claim

  • Receipt of Consultation on the same day is required for the reimbursement of the cost of medication under consultation at Physician’s office, Specialist’s consultation or Chinese Medicine Practitioner’s treatment benefits

* For Specialist’s consultation, referral letter for Dermatologist, Ophthalmologist, Gynaecologist, Orthopaedist & Traumatologist, Otorhinolaryngologist (ENT) and Paediatrician are waived, unless otherwise specified in the policy.

For Panel Services
The Insured can simply present FWD Healthcare card and pay the co-payment (if any) stated on the card at the designated clinics to receive consultation/treatment. There is no need to submit a claim request.

Submission Timeline

  1. Please submit the completed claim form with required supporting documents to us within 90 days from the date of consultation/treatment.

* Only applicable to CARING Family Medical Insurance Plan.
